Missed Appointments

Your appointment time is reserved specifically for you and only you. We understand that unplanned issues can come up and you may need to cancel your appointment. If this happens, we respectfully request that you cancel scheduled appointments at least 24 hours in advance.

Our doctors want to be available for your needs and the needs of all our patients. When a patient does not show up for a scheduled appointment, another patient loses the opportunity to be treated.

Be advised, a $50 fee may be added to the patient’s account if we do not receive a notification to cancel an appointment at least 24 hours in advance. This fee will be collected before the next appointment. If the patient or guardian presents a written doctor’s note, the fee will be waived. After 2 consecutive missed or canceled appointments, we reserve the right to dismiss the patient.

Late Arrivals

Patients who arrive 15 minutes late or more may need to be rescheduled. New patients who have not completed their new patient forms prior to their appointment may need to be rescheduled. We will try our best to accommodate your needs, however, we also need to respect other patients’ time.

Patient Code Of Conduct

All patients, staff, and guests are expected to conduct themselves in a respectful and courteous manner. Words or actions that are disruptive, disrespectful, racist, discriminatory, hostile or harassing are not welcome and will be addressed in a manner that protects the safety and dignity of all involved. If we believe you have violated the policy with unwelcome words or actions, you will be given the chance to explain your point of view. We will always carefully consider your response before we make any decisions about future care with our practice.

Form Fee Guidelines

Our office may charge a fee for the completion of certain forms which require medical information and/or a physician’s signature. The fee ranges from $40-$65 depending on the length and required detail. Pre-payment is required.

Notice of AI Use

Our office may use artificial intelligence (AI) tools to help with things like note-taking, scheduling, or support in your care. These tools are only assistants – your doctor and care team make all medical decisions. Your information is always kept private and secure.
