Initial Consultation

Your initial appointment will consist of a consultation. Our doctor will evaluate you, explain your diagnosis and treatment options, and answer any questions you may have. Please complete your new patient forms at least 24 hours before your appointment. New patients should plan to arrive 10 minutes early for your appointment and bring the following information with you:

  • A list of current medications
  • Any eye drops that you are currently using
  • Insurance card(s)
  • Valid driver’s license or other photo ID

IMPORTANT: All patients under 18 must be accompanied by a parent or guardian at the consultation visit.

The charges for an initial consultation can vary, depending on the nature of the visit and the services provided. For some patients there may only be a charge for the consultation itself, whereas other patients may have additional charges for other services such as diagnostic testing or procedures. Payment is due at the time of service, regardless if insurance is involved or not. Cosmetic services are not billable to insurance.

If your doctor has referred you, he or she will promptly receive a letter outlining our findings and the course of treatment. We encourage you to return to your referring doctor after our specialized care is complete.
